The form will ask you about your medical history, which is extremely important when choosing to undergo a surgical or even a non-surgical procedure. It’s also important to let Dr. Gardenier know about any medications you might currently be taking.

Financing your cosmetic procedure can be stressful, but we help to take the strain out, providing all the information and support you could need to move forward. We work with a variety of healthcare lenders that includes CareCredit, M-Lend Finance, Advance Care Card, and MyMedicalFunding, many of which can provide instant approval. These lenders have reasonable repayment terms, APR’s and down payments as well.
